  • 401(k) Flyer from Wells Fargo – information about Waggoners trucking 401(k) Program administered by Wells Fargo.

  • Direct Deposit Form – to establish new direct deposit or make changes to your existing direct deposit.

  • EBMS Benefits Enrollment Form – to sign up for coverage under EBMS

  • EBMS Removal of Dependent Coverage Form – to remove existing dependents currently covered by EBMS.

  • Employee Emergency Contact Information – to indicate who Waggoners should contact on your behalf in case of an emergency.

  • Leave of Absence Request Form – to request vacation, sick leave, or other paid / unpaid leave from the Company.

  • W-4 – to change your dependent exemptions for I.R.S. purposes.
