    Waggoners Trucking uses Employee Benefit Management Services, Inc. (or EBMS) as our health plan Contract Administrator.

    All full-time regular employees and their dependents are eligible to participate in The Waggoners Trucking Group Health and Dental insurance program. Coverage for employees and their dependents is effective on the first day of the month following 90 days of continuous employment.

    Group Health and Dental enrollment forms are provided during your orientation for you and your eligible dependents.

    Upon successful enrollment, you will receive EBMS. Insurance Cards and other literature pertaining to our Group Health and Dental Insurance benefits.

    No Open Enrollment Policy
    The Waggoners Trucking Group Health and Dental Plan does not allow for open benefits enrollment. If you decline enrollment for yourself or your eligible dependants because you, or they, are covered by another insurance program, you may not be able to enroll in Waggoners program in the future unless you have a new dependant as a result of marriage, birth, adoption, or placement of adoption.

    You may be able to enroll yourself and your dependants provided that you request enrollment within thirty days after the marriage, birth, adoption, or placement of adoption.

    Prescription Benefits Program

    All eligible employees are mailed literature pertaining to our Prescription Benefit Program upon enrollment in our Group Health and Dental Insurance Program. Your EBMS insurance card also acts as your prescription card.

    Certain medications may not be covered under the Prescription Benefit Program. Refer to your benefit plan documents for more information.

    Health, Dental & Prescription Plan Q & A

    Question: What is the waiting period for Group Health, Dental and Prescription Benefits?

    Answer: Participant coverage under the Plan shall become effective with respect to an eligible person on the first of the month following 90 days of continuous employment.

    Question: Can I enroll my family at anytime after I have completed 90 days of continuous employment?

    Answer: No, we do not have an Open Enrollment Period. You may enroll other eligible family members only if you have a qualifying event such as; Marriage, Adoption, Birth of a Child, Death

    Question: Can I see any Doctor, dentist or clinic?

    Answer: Our Group Health Plan is covered under a preferred provider organization commonly known as a “PPO”. A complete list of eligible Doctor’s, Clinic’s and Hospitals are available on-line at www.beechstreet.com. Our Dental Plan is not covered under a PPO, therefore you are may use any Dentist for your Dental needs.

    Question: Who is our Group Health, Dental and Prescription Drug Insurance Carrier?

    Answer: We provide Benefits to all employees through Waggoners Trucking self-funded Health plan. Employee Benefit Management Services (EBMS) in Billings, MT is our Third Party Administrator who manages our Group Plan.

    Question: Who should I call I have questions regarding my Health, Dental or Prescription Plan.

    Answer: You may call the EBMS Customer Service center at 1-866-462-9050. Prescription questions call 866-894-1497 or on-line at http://www.ebms.com then click on online services and then employee.

    Question: Is Vision coverage available?

    Answer: No, not at this time.

    Long Term Disability

    The Waggoners Trucking provides all full-time regular employees with long-term disability insurance benefits. Coverage for eligible employees is effective on the first of the month following 90 days of continuous employment.

    After 90 days of continuous employment you are automatically enrolled without charge. Our coverage is through Mutual of Omaha. This benefit pays 60% of your regular salary after you have established a disability for a period of 90 days.

     

    Life Insurance

    All full-time regular employees are eligible to participate in The Waggoners Trucking life insurance program. Coverage under this program is available for employees only; dependent coverage is not available. Coverage for eligible employees is effective on the first day of the month following 90 days of continuous employment.

    You may enroll in the life insurance program by completing an Employee Life Insurance Application during orientation or by obtaining an application form from the Personnel Department in Billings.

    The amount of Life Insurance is $20,000 for our Driver personnel and one times annual salary for our administrative personnel.

    Life Insurance benefits terminate on the date your employment with The Waggoners Trucking terminates. For further details regarding life insurance benefits, consult the summary plan description booklet in this handbook or contact the Personnel Department in Billings.

     

    401K Retirement Plan

    The Waggoners Trucking has established a retirement program to assist you in your post-employment years. You are encouraged to begin planning for retirement early in your career so Waggoners Trucking benefits, along with personal investments and government programs, can grow together to provide future financial security.

    The 401 (k) retirement plan is funded by both employee and Waggoners Trucking contributions and all employees are eligible to participate. Your participation (if you elect) begins on the first day of the month following six months of continuous employment.

    Question: When can I participate in the Waggoners Trucking 401(k) Retirement Plan?

    Answer: After six (6) months of continuous employment. You will receive enrollment information from Wells Fargo Institutional Investments prior to your participation date.

    Question: Does the Company offer a matching contribution to the 401(k) Retirement Plan?

    Answer: Yes, the Company matches dollar for dollar up to 3% of your deferral percentage, then contributes an additional ½% up to 4% of your deferral and additional ½% up to 5% of your deferral for a total match of 4%.

    Question: What percentage of my income may I contribute to the 401(k) Retirement Plan?

    Answer: You may defer 1% to 100% not to exceed the maximum annual limit of $16,500.00 in 2010.

    Question: Can I change my deferral percentage?

    Answer: Yes, you can change your deferral percentage at the beginning of each month by contacting a Wells Fargo Representative at 1-800-258-2715 or access on-line at http://retirementplan.wellsfargo.com.
